How do brains create all our different colors, pains, and other conscious qualities? These various qualia are the most essential aspects of consciousness. Yet standard neuroscience (primarily based on synaptic information processing) has not found the synaptic-firing codes, sometimes described as the “spike code,” to account for how these qualia arise and how they unite to form complex perceptions, emotions, et cetera. Nor is it clear how to get from these abstract codes to the qualia we experience. But electromagnetic field (...) (versus synaptic) approaches to how qualia arise have been offered in recent years by Pockett, McFadden, Jones, Bond, Ward and Guevera, Keppler and Shani, Hunt and Schooler, et cetera. These EM-field approaches show promise in offering more viable accounts of qualia. Yet, until now, they have not been evaluated together. Physical pain has always been part of human experience, and throughout history it is recorded that doctors and wise men and women have sought to ease pain. The attitudes of those suffering pain, however, have varied from stoical acceptance to sullen endurance. Today, most people consciously seek to avoid pain or to have their pain eased, although they do not always expect what in fact appears to be possible. This study of 13 patients with protracted pain was carried out at (...) The London Hospital by a professional group to see how patients regarded their own pain and the efforts of doctors and nurses to relieve it. Current methods of obtaining an informed consent leave much to be desired. Patients rarely read consent forms or understand all of the risks, benefits, or alternatives associated with their treatment. Evaluating the advantages and disadvantages of treatment options often presents a more significant challenge for patients with lower levels of health literacy. This article reviews the evidence of shortcomings in our informed consent system and then explores the potential for a new approach to engage patients at all levels of health (...) literacy in their treatment decisions. Specifically, the article will examine the potential of shared decision-making to bridge gaps in knowledge, increase patient adherence to treatment, and improve health outcomes in low health literacy patient populations. Leveling barriers to treatment information for disadvantaged populations should be a public health imperative, especially if it can be shown to improve health outcomes and reduce health disparities. BackgroundUnderstanding sex and gender in health research can improve the quality of scholarship and enhance health outcomes. Funding agencies and academic journals are two key gatekeepers of knowledge production and dissemination, including whether and how sex/gender is incorporated into health research. Though attention has been paid to key issues and practices in accounting for sex/gender in health funding agencies and academic journals, to date, there has been no systematic analysis documenting whether and how agencies and journals require attention to sex/gender, (...) what conceptual explanations and practical guidance are given for such inclusion, and whether existing practices reflect the reality that sex/gender cannot be separated from other axes of inequality.MethodsOur research systematically examines official statements about sex/gender inclusion from 45 national-level funding agencies that fund health research across 36 countries (covering the regions of the EU and associated countries, North America, and Australia) and from ten top-ranking general health (the top five in “science” and the top five in “social science”) and ten sex- and/or gender-related health journals. We explore the extent to which agencies and journals require inclusion of sex/gender considerations and to what extent existing strategies reflect state of the art understandings of sex/gender, including intersectional perspectives.ResultsThe research highlights the following: (a) there is no consistency in whether sex/gender are mentioned in funding and publishing guidelines; (b) there is wide variation in how sex/gender are conceptualized and how researchers are asked to address the inclusion/exclusion of sex/gender in research; (c) funding agencies tend to prioritize male/female equality in research teams and funding outcomes over considerations of sex/gender in research content and knowledge production; and (d) with very few exceptions, agency and journal criteria fail to recognize the complexity of sex/gender, including the intersection of sex/gender with other key factors that shape health.ConclusionsThe conceptualization and integration of sex/gender needs to better capture the interacting and complex factors that shape health—an imperative that can be informed by an intersectional approach. This can strengthen current efforts to advance scientific excellence in the production and reporting of research. We provide recommendations and supporting questions to strengthen consideration of sex/gender in policies and practices of health journals and funding agencies. (shrink)

Daniel Callahan has argued that economic and social benefits would result from a policy of withholding medical treatments which prolong life in persons over a certain age. He claims 'the real goal of medicine' is to conquer death and prolong life with the use of technology, regardless of the age and quality of life of the patient, and this has been responsible for the escalation of health care expenditure. Callahan's proposal is based on economic rationalism but there is little evidence (...) to suggest that substantial economic savings could be achieved. Moreover, his argument raises serious moral objections. A policy of withholding treatments from members of a social group involves elements of compulsion and discrimination, both of which would intrude on the doctor-patient relationship, undermine the autonomy of elderly patients, and invoke the slippery slope towards involuntary forms of euthanasia. Life-death decisions should be based on more than the one criterion of age, and take account of more relevant factors such as the patient's usual state of well-being, her/his expressed wishes, informed consent and the type of illness. Any move to the implementation and enforcement of the policy Callahan recommends would be rejected by health professionals and the public. (shrink)

Where is consciousness? Neurobiological theories of consciousness look primarily to synaptic firing and “spike codes” as the physical substrate of consciousness, although the specific mechanisms of consciousness remain unknown. Synaptic firing results from electrochemical processes in neuron axons and dendrites. All neurons also produce electromagnetic (EM) fields due to various mechanisms, including the electric potential created by transmembrane ion flows, known as “local field potentials,” but there are also more meso-scale and macro-scale EM fields present in the brain. The functional (...) role of these EM fields has long been a source of debate. We suggest that these fields, in both their local and global forms, may be the primary seat of consciousness, working as a gestalt with synaptic firing and other aspects of neuroanatomy to produce the marvelous complexity of minds. We call this assertion the “electromagnetic field hypothesis.” The neuroanatomy of the brain produces the local and global EM fields but these fields are not identical with the anatomy of the brain. These fields are produced by, but not identical with, the brain, in the same manner that twigs and leaves are produced by a tree’s branches and trunk but are not the same as the branches and trunk. As such, the EM fields represent the more granular, both spatially and temporally, aspects of the brain’s structure and functioning than the neuroanatomy of the brain. The brain’s various EM fields seem to be more sensitive to small changes than the neuroanatomy of the brain. We discuss issues with the spike code approach as well as the various lines of evidence supporting our argument that the brain’s EM fields may be the primary seat of consciousness. This evidence (which occupies most of the paper) suggests that oscillating neural EM fields may make firing in neural circuits oscillate, and these oscillating circuits. may help unify and guide conscious cognition. Brian Ellis has argued that the assigning of forces is, in the final analysis, a matter of convention. This conclusion is backed by the premises (1) that forces and force-effects are necessary and sufficient for each other, and (2) that the classification of some state of affairs as a force-effect is at least partly conventional. We argue that the first premise is false, that the second premise is ambiguous as between several senses of "conventional," and finally that he has not (...) established that force-effects are conventional in the sense required for the conclusion he wishes to draw. U.S. immigration policies and enforcement can make immigrants fearful of accessing healthcare. Although current immigration policies restrict enforcement in “sensitive locations” including healthcare facilities, there are reports of enforcement actions in such settings.

Kenneth Goodpaster has criticized ethicists like Feinberg and Frankena for too narrowly circumscribing the range of moral considerability, urging instead that “nothing short of the condition of being alive” is a satisfactory criterion. Goodpaster overlooks at least one crucial objection: that his own “condition of being alive” may aIso be too narrow a criterion of moral considerability, since “being in existence” is at least as plausible and nonarbitrary a criterion as is Goodpaster’s. I show that each of the arguments that (...) Goodpaster musters in support of his criterion can be used equally weIl to bolster “being in existence” as a test of moral considerability. Moreover, I argue that “being in existence” appears to be a stronger criterion overall, since it is broader. Until or unless a fuller justification is forthcoming of “being alive” as a satisfactory criterion of moral considerability-a justification which must demonstrate that “mere things,” included under the condition of “being in existence,” do not deserve moral consideration--Goodpaster’sthesis is confronted with a serious problem. The authors empirically examine the nature and extent of ethical problems confronting senior level AICPA members (CPAs) and examine the effectiveness of partner actions and codes of ethics in reducing ethical problems. The results indicate that the most difficult ethical problems (frequency reported) were: client requests to alter tax returns and commit tax fraud, conflict of interest and independence, client requests to alter financial statements, personal-professional problems, and fee problems. Analysis of attitudes toward ethics in the accounting profession indicated that (...) (1) CPAs perceive that opportunities exist in the accounting profession to engage in unethical behavior, (2) CPAs, in general, do not believe that unethical behavior leads to success, and (3) when top management (partners) reprimand unethical behavior, the ethical problems perceived by CPAs seem to be reduced. John Blund's Treatise on the Soul is probably the earliest text of its kind: a witness to the first reception of Greek and Arabic psychology at Oxford and foundation for a new area of medieval philosophical speculation. This book contains Hunt's Latin edition with a new English translation and a new introduction to the text by Michael Dunne.

This treatise was written c. 1200, and is the earliest known philosophical work by an Oxford master. Its great interest is that it demonstrates the way in which the first generation of scholars used the translations of Greek and Arabic philosophical scientific texts which had just become available in Western Europe.

The world I grew up in believed that change and development in life are part of a continuous process of cause and effect, minutely and patiently sustained throughout the millenniums. With the exception of the initial act of creation ..., the evolution of life on earth was considered to be a slow, steady and ultimately demonstrable process. No sooner did I begin to read history, however, than I began to have my doubts. Human society and living beings, it seemed to (...) me, ought to be excluded from so calm and rational a view. The whole of human development, far from having been a product of steady evolution, seemed subject to only partially explicable and almost invariably violent mutations. Entire cultures and groups of individuals appeared imprisoned for centuries in a static shape which they endured with long-suffering indifference, and then suddenly, for no demonstrable cause, became susceptible to drastic changes and wild surges of development. It was as if the movement of life throughout the ages was not a Darwinian caterpillar but a startled kangaroo, going out towards the future in a series of unpredictable hops, stops, skips and bounds. Indeed, when I came to study physics I had a feeling that the modern concept of energy could perhaps throw more light on the process than any of the more conventional approaches to the subject. It seemed that species, society and individuals behaved more like thunder-clouds than scrubbed, neatly clothed and well-behaved children of reason. Connections between W. James and L. Wittgenstein have been widely highlighted in recent scholarship: his mature reflections on the philosophy of psychology found in James a major source of inspiration. This paper gives reason of Wittgenstein's refusal to being labelled "pragmatist" and stresses -against Schulte- the influential role of James in the development of Wittgenstein's thought.

We find before us an excellent edition of the book which the influential American thinker Ralph Waldo Emerson (1802-82) published in December of 1860, four months before the outbreak of the American Civil War. The central question which Emerson poses in this volume concerns the conduct of life, that is, of how to live. The titles of the nine essays, which compose the book, illustrate the themes tackled: “Fate,” “Power,” “Wealth”, “Culture,” “Behavior,” “Worship”, “Considerations by the Way,” “Beauty” and “Illusions.” (...) As Callaway suggests, Emerson’s is not a philosophy in the sense of contemporary technicalities, “the basic tendency of his thought is a metaphysical idealism in which the soul and intuition or inspiration are central.” (p. xvi). As an essentially religious thinker, profoundly preoccupied with the human soul and with the development of human potentialities, he has always firmly opposed to slavery: one cannot refuse to others human beings the development of their distinctively human potentialities (p. xxvii).
